How to write host home notes
Host home notes are due every 14 days. There has to be a log for each day of the year. January 1 notes have to be turned in by January 14 and January 2 notes have to be turned in by January 15, etc. The comments and descriptions, as set as an example in the documents below, are no longer necessary. Only check-marks are needed unless specifically mentioned by your case manager that they are necessary for the client.

In-Home ISS is specifically for clients with medical needs and have provided the necessary documentation to the case manager and the service is added on the IPC. An in-service training is required by each staff performing the in-home ISS and must be set with your assigned case manager. Please reference the PowerPoint presentation below and if you have any further questions, ask your case manager
