Steps to Begin Dental Services
- Dental service is added to your family member's IPC for the plan year ($2,000 for HCS; $1,000 for TxHmL)
- Locate Dental office of your choice (in the list below) & set an appointment
- Mention that you are from A Meaning of Life
- Send Case Manager's (CM) email and phone number
- AMOL will send letter of authorization (LOA) confirming that funding is available
- Go to your appointment!
